After COVID-19 infection how long does a patient carry live and infectious virus? No one knows the answer to this question and there are at least 200 publications coming from different groups across the globe addressing different aspects of this question. This resulted in variations in quarantine and segregation across the globe changing from 5 days to 30 days in extreme cases. The above-mentioned publications have also shown that length of infection varies tremendously between patients depending on their immune status. Although phenomenal progress has been made during the COVID-19 pandemic in the field of diagnostics, to date, no molecular diagnostic test can reliably differentiate between live virus and RNA fragments which are harmless souvenirs of an infection long past.
Subgenomic RNA has long been suggested as a marker for active replication in SARS-CoV-2 infection. However, mere detection of subgenomic RNA in the samples does not correlate with active replication. By developing an unique assay that includes patent pending innovations in primer/probe design and an unique data analysis algorithm, we have been successful in identifying samples that are positive for infectious virus. Detecting the presence of active replication and infectious virus can be used in clinical trials using antivirals small molecule drugs as well as blocking antibodies. Subgenomic mRNA active replication test (SMART) kit is a "Research Use Only (RUO)" three color RT-qPCR assay that can be performed using any real-time PCR machine capable of detecting FAM, HEX and Cy5 channels. SMART is compatible with RNA extracted from nasal swabs using any of the established methods.
